WHAT YOU NEED TO KNOW ABOUT NECK AND FACE LIFTING.
It's the natural repositioning of the muscles and adaptation of the skin on the face and neck.
It is most frequently performed to surgically correct aging and restore the younger appearance of your face.
THE GOAL
The goal of such an operation is not to change the shape or appearance of the face.
On the contrary, by simply restoring the various anatomical structures of the face and neck (skin, muscle, fat), the surgeon can return to his or her former appearance.
Who is a proper candidate?
As with all surgeries it is of high importance to be in a good overall health in the first place. However, the condition of the skin is specifically important for facelift surgeries. Skin should be flexible and elastic.
These features might be affected by alcohol consumption and smoking. If this is the case, patients should consult health specialists about a realistic outcome of a facelift.
People with chronic illnesses such as diabetes, heart and blood clotting disorders are eligible for the procedure.
However, such conditions can affect a person’s suitability for undergoing anaesthesia and can also disrupt wound healing during the recovery process.
It is important to discuss the particular case with a surgeon so that any complications can be prevented and the desired look achieved. The facelift procedure can and should be adjusted to bring the most satisfying results.
Facelift techniques
There are several techniques and types of facelift, and the exact choice should be specifically tailored to fit the individual case.
The patient and the surgeon should discuss the scope of the procedure, its degree of invasiveness, and how dramatic the results are expected to be so that the best technique can be chosen to fulfil the patient’s expectations.
The minimal access cranial suspension (MACS)
Also known as short-scar surgery, is more suitable for patients with good skin tone and limited tissue sagging. It is performed under local anaesthesia and the scar is kept minimal by making incisions in front of each ear from the hairline to the earlobe
MACS facelift is designed to affect the lower and middle thirds of the face. As it is not possible to remove a lot of skin during this procedure it is usually recommended for younger patients with good skin elasticity
It is considerably less undermining; however, as facial muscles are not tightened the results are not as long-lasting as with the SMAS technique.
The superficial musculoaponeurotic system (SMAS)
facelift is better suited for patients with moderate to severe laxity and sagging of the facial muscles, ligaments and skin.
These signs of ageing appear when facial ligaments lose their elasticity and can no longer protect facial skin from gravitational effects.
During SMAS facelift the surgeon will tighten the underlying facial muscles using multi-vector pull, i.e. pulling the tissues in several directions for the best contour of cheeks and jawline. Any excess skin will be removed.Note that this procedure requires general anaesthesia. And the results are much more lasting than after the MACS lift.
The deep plane lift technique
The deep plane lift technique is designed to fix jowls, “turkey gobbler neck”, and laugh lines. The surgeon makes an incision around the ears, separates the musculoaponeurotic system and repositions it by plicating and tightening loose muscles of the neck and the middle part of the face.
Deep plane lift allows for an extremely natural appearance and long-lasting results.
What is the difference between a deep plane facelift and a SMAS facelift?
The deep plane facelift technique is sort of a little step forward from the classical SMAS facelift, but the main idea still remains the same – SMAS layer release and lifting. Therefore, if one asks whether the deep plane facelift still is a SMAS facelift, the answer is yes.
There is no other way to perform a facelift than manipulating with the SMAS layer.
The only difference is that the deep plane facelift is more precise and focuses nearly only on deeper layers of the skin where the actual problem lies, barely raising the superficial layer (i.e. subcutaneous tissue)
It is a straight-to-the-point facelift that does little tissue damage near the skin surface, which results in less bruising and swelling, as well as in less downtime after the surgery
Preparation :
- A patient might have very specific expectations for the outcome of the surgery, therefore a thorough discussion with a surgeon is necessary in order to select the most suitable method.
- Skin condition greatly depends on nutrition. Therefore, the patient should ensure that he or she is consuming enough proteins (which are crucial for wound healing) and vitamins such as zinc, vitamin A and C, which are important for the synthesis of collagen and for maintaining skin integrity. Note that vitamin, mineral and protein malnutrition impairs all stages of immune response, wound healing and tissue production.
-I t is essential to talk to your doctor a couple of weeks before the surgery as you might be instructed to stop taking medications such as aspirin or other blood-thinning drugs.
- Smoking and alcohol consumption should be avoided one month before and after the surgery so that it does not affect the healing process by restricting blood circulation.
- Medical examination and a review of medical history is an essential step before the surgery. In order to determine the best facelift type a surgeon will examine the patient’s face and will discuss the possible strategies. Patients will have to undergo any necessary lab tests, including a blood work, a urine test, ECG (electrocardiogram), X-rays, or others. The surgeon will explain in detail the plan of the surgery and recovery.
In order to prevent regurgitation during the surgery an empty stomach is required for anyone undergoing general anaesthesia. Therefore, patients should not eat or drink one day before the surgery.
Facelift and Eyelid surgery
For even more expressed results it is possible to combine a facelift with other procedures, including neck lift, eyelid lift or rhinoplasty (nose job).
Possible complications
Hematoma is a result of ruptured capillaries. It is not dangerous and will fade away within a few weeks after the surgery. Seroma is a build-up of body fluids under the skin at the site of the surgery. Seromas do not pose any threat and should resolve themselves in a few weeks’ time.
Infection and delayed healing can occur if the doctor’s recommendations for a successful recovery are not followed.
A slight reduction in hair or hair growth near the incision sites is also possible. Facial nerve damage is extremely rare if surgery is performed by an experienced plastic surgeon
After the operation
After the surgery is performed, it can take up to four weeks to fully recover. The patient should take time off work for the whole period of recovery.
Bandages are used for covering the wounds, while an elastic facial sling prevents swelling and ensures equal distribution of pressure. If serous fluid drainage tubes are used they will be removed in several days, as well as the stitches (unless dissolvable stitches were used).
Bruising should fade away in a couple of weeks. However, to be able to appreciate the final results of the procedure the patient might need to wait from six to nine months until the swelling has fully gone down.
Sun should be avoided for several weeks as it can interfere with scar healing and damage the surrounding vulnerable skin. It is highly recommended to avoid driving after the surgery at least until bruising is relieved.
The patient should also avoid wetting the bandages for a couple of days. Strenuous physical activity should be ceased for at least two weeks.
The patient should resist applying makeup while the wounds are still healing.
Quality skin care is required to prevent early ageing and it inevitably adds to the effect of a facelift.
Weight fluctuation tends to stretch the skin, hence a healthy diet and a stable weight can prolong the results.
